All equipment is to be Owned Free and Clear for our Option “A” Program

Note the: quantity, age, manufacturer/name   make/model, description of the piece, list the serial number, (you do
not have to list the serial numbers at this time), Note the $ Value is: today’s approximate value for each item.

Add the total dollar amount of each page, and arrive at a grand total then FAX BACK: the Application, the Credit
Release form, and the Equipment List form.
